Pre-Employment Requirements

When you receive an employment offer, you will be required to complete the following prior to attending hospital orientation:

Drug testing
A pre-placement health screen
Tuberculosis blood test
Chest x-ray (if known positive tuberculosis skin test)
Rubella immunity (German measles)
Rubella immunity (measles)
Varicella immunity (chicken pox)
Hepatitis B vaccine (if occupation requires patient contact)
N-95 respirator mask fit testing (if occupation requires patient contact)

Frequently Asked Questions

What paperwork do I need to complete prior to my first day of orientation?

Human Resources will send you to Occupational Health for your initial paperwork and drug testing. At that time Occupational Health will schedule your health screen and completion of the health requirements. You will be asked to complete a criminal record release form. If you have been hired into a position that requires a license or certification, your credentials will be verified. You will need to complete a W4 form for taxes, bring in your license plate number for your parking pass and sign a confidentiality agreement. You will also have the option of signing up for direct deposit. If you are interested in the direct deposit option, you will need to provide us with your banking information, as well as a voided check.

What information will I need to provide to show my eligibility to work?

You must provide original documentation that establishes your eligibility to work in the United States according to Federal Law. You will also be required to undergo a criminal background check, pre-employment health screening, reference checks and verification of certification, licensure and/or registration. If your position requires driving a motor vehicle, you will also undergo a motor vehicle record check.

Am I required to attend orientation?

All employees (full-time, part-time and per diem), contract workers, students and volunteers are required to attend Hospital Orientation prior to their first day of work.

Select, clinical staff with direct patient care responsibilities will also be required to attend a clinically-focused orientation.

When is orientation?

Hospital orientation is held twice a month (one day session and one evening session). The orientation program is five hours in length. A light fare is served at both sessions.

Where does orientation take place?

Directions to hospital orientation will be provided to you by Human Resources.

What do I wear?

Business casual is acceptable for orientation. Please keep in mind that jeans, shorts and similar types of clothing are not considered appropriate for business.

Where do I park when I come in for my interview?

You should park in the visitor parking area directly across the street from the hospital.

Where should I go for my interview?

When you are called to schedule your interview, you will be advised at that time where you should report for your interview.
