Obtaining Your Test Results and Records Electronically

Adults:

  • Provide your e-mail address during registration to receive portal invitation to set up an account. After your results are published to the portal, you may print a copy.
  • Visit patientportal.lawrencegeneral.org and enter the required information to set up your account.

Minors 13-17 Years of Age:

  • Minors between 13 and 17 years of age may have portal access for their own results by registering directly for the patient portal using their own email address. Should a minor want to share this access with a parent or guardian, they may do so within the portal application under the "Share" feature. Limited information can be designated for the share feature such as patient billing only.
  • Visit patientportal.lawrencegeneral.org and enter the required information to set up your account.
  • Patient portal results are now available in compliance with the Federal 21st Century Cures Act. If there are any urgent findings that need immediate attention, your health care provider has already been notified. If you have questions about your findings, we encourage you to discuss these at your next planned visit, after your ordering provider has had time to review your test results.
  • Duplicate email addresses cannot be used. Each patient must have their own unique email address.

Children UNDER 13 Years of Age:

  • Parents with children under 13 years of age must visit patientportal.lawrencegeneral.org and complete the enrollment request for their child. A proxy form (you can download here) will be sent either electronically or in hard copy depending upon preference and must be completed by the parent/guardian for all children under 13 years of age. Once this is submitted and processed, you may access your child’s portal account to print documentation of results.

Result times vary day-to-day and depend on many factors. Please allow 5 days prior to traveling to receive documentation of results.

Obtaining a Copy of Your Personal Medical Record

All requests for medical information from Lawrence General Hospital are directed to the Health Information Services Department. Requests for records should be made on the “Authorization to Use or Disclose Protected Health Information” form. 

Once you have completed the form, return it to the Health Information Services Department by:

Requests will be processed within 5-7 business days of receipt. Please note, there may be a reasonable fee for copying records.

Obtaining a Copy of Your Baby’s Birth Certificate

Massachusetts hospitals do not retain legal copies of birth certificates. Requests for legal copies should be directed to:
